HOPA Member Awards
HOPA members can nominate themselves or others in honor of their work advocating, supporting, and advancing hem/onc pharmacy. Nominators must be HOPA members in good standing. Self-nominations are available for eight of the 13 awards.

FHOPA
The HOPA Fellow program recognizes excellence in hematology/oncology pharmacy. The Fellow designation (FHOPA) is awarded to individuals who have made sustained contributions to HOPA and who have also demonstrated exceptional performance in the field.
How to Nominate
Login to the Hematology/Oncology Pharmacy Association portal with your email and password.
Create your online submission.
During the Call for Nominations period, your application can be saved as DRAFT until all required information is completed and attachments uploaded.
On completion, save your Application as FINAL and download a copy for your records.

Early Career Mentorship Program
HOPA's legacy program is designed to support early career members through guided mentorship and leadership development. The HOPA Mentorship Program pairs early-career practitioners with mentors to help improve professional competencies and leadership skills.
This 9-month program represents a bridge between senior leaders in hem/onc pharmacy and up-and-coming hem/onc pharmacists. Limited spots available. Please review program criteria below. Applicants will be notified in January 2027.
Eligible Mentees
- Have been in practice for at least three years (PGY2 year may be included)
- Complete an application form
- Submit a CV
- Provide contact information for two recommendation sources
Eligible Mentors
- Past HOPA Board members or HOPA Fellows
- Complete a short interest survey
